An estimated 60% to 84% of patients with cancer develop bone metastasis. Of these 70% experience pain syndrome which is difficult to manage, of which 50% die without adequate pain relief with a poor quality of life. It is therefore necessary to have accessible and effective medications for the management of this condition. One of the most common pain syndromes in patients with advanced cancer is bone metastasis. This is difficult to manage and control in clinical practice. Currently, scientific advances in cancer detection and treatment have prolonged life expectancy in patients. Unlike the case with the phenomenon of bone pain in cancer, where current treatment strategies are not significantly effective. Most palliative treatment of bone pain are based on clinical studies on pain management in patients or in experimental models is not well designed this could explain why the drugs used are partially effective. Today, one of the main obstacles in developing new, safe treatments to control bone pain is the absence of basic science knowledge in the physiology of bone pain.

Epidemiology
The pain in cancer patients is usually multifactorial, may arise from the process itself, treatment side effects or both. For these reasons the approach and management of this symptom should be multidisciplinary. Pain syndrome occurs either by local proliferation or tumor invasion of a metastatic tumor from a distance. With metastatic bone pain often reflects the presence of a tumor in breast, thyroid, prostate, kidney, lung or adrenal.
Physiology of bone pain
Bone pain is associated with tissue destruction by osteoclast cells. Normally, osteoclastic bone resorption are in balance with bone formation mediated by osteoblasts. In neoplastic osteolytic activity is increased and there are substances such as cytokines, local growth factors, peptides similar to parathyroid hormone and prostaglandins. Autacoids are also released other owners as potassium ions, bradykinin and osteoclast activating factors. These tissue substances play an important role in sensitizing the neural tissue against chemical and thermal stimuli, lower thresholds for discharge of the neuronal membrane, produce exaggerated responses to stimuli above the threshold and result in discharges of tonic impulses normally silent nociceptors. This phenomenon is called peripheral sensitization and primary hyperalgesia and is understood as events occurring within the ranks of the injured tissue and stimulate peripheral nociceptors (C fibers and A delta fibers) translating pain. In bone tissue of the sensory receptors are located primarily in the periosteum, whereas the bone marrow and bone cortex are insensitive. This phenomenon of peripheral sensitization results in abnormal sensitivity to pressure surrounding skin (allodynia and hyperalgesia), pain in muscles, tendons, joints and deep tissues in contact with bone. This is limited to ensure that the peripheral ends have a greater capacity for alarm response to injury.
The constant presence of harmful process, stimulating nociceptive receptors gives the introduction of a subacute pain that tends to be chronic with the growth of bone metastases. These stimuli lead to another prevalent phenomenon called central sensitization important which includes abnormal amplification of incoming sensory signals to the central nervous system, particularly the spinal cord. The phenomenon occurs because of the persistent input stimulus through the fibers C. This spinal cord triggers a temporary increase in the power of silent synaptic terminals. In this process plays an important role of glutamate receptor N-methyl-D-aspartate (NMDA). The resulting amplification of the signal generated in the postsynaptic neuron sends a message to the brain which is interpreted as pain. In short central sensitization amplifies the sensory effects of both peripheral nociceptive inputs (C fibers of pain) and non-nociceptive fibers (A of touch).
In practice the two phenomena come together in the genesis of metastatic bone pain and peripheral sensitization occurs acutely metastatic lesions to appear nociceptors and translate the information conveyed through the afferent myelinated A-delta or unmyelinated C fibers to the spinal cord where the information is modulated by various systems. With the set up process subacute begins the process of central sensitization which sensory synapses begin to activate silent. And there is a state of increased central perception. By becoming chronic pain phenomenon becomes even more complex because all that is in contact with the area of injury becomes a powerful generator of pain. The touch, muscle movement or joint pain result, manifesting the phenomena of allodynia and hyperalgesia much more marked.
With progression and growth of metastatic disease can appear phenomena of compression of peripheral nerves, nerve roots or spinal cord. Then the pain can refer to other dermatomes, further complicating the initial picture painful. This condition becomes a debilitating factor for the patient and to be inadequately controlled could trigger the phenomenon of total pain detailed below.

What is Chemo Brain?

Chemo brain comes as a surprise to many cancer patients. Survivors think their lives will get back to normal when the cancer is gone. Unfortunately, as many as 50% of chemo patients report a lingering phenomenon known as chemo brain. The effects can stonewall attempts to get back to their day-to-day lives. What is chemo brain? What are the symptoms? What can you do about it?

Some patients report mental cloudiness after chemo treatments. Therefore, chemo brain also goes by the name 'chemo fog.'

Not all cancer patients suffer chemo brain. A patient may have two choices to fight cancer. The two choices are chemo or surgery, depending on a number of factors. Chemo uses chemicals to kill cancer cells. Cancer patients treated with surgery do not appear to get this mental cloudiness. At least not at the level chemo patients experience.

Fear of this cognitive dysfunction can influence the course of treatment.

There has been a debate for years. Some medical professionals do not believe it exists. Of those who do believe chemo brain is real, they do not agree on the cause.

Recently have there been studies on chemo brain. They are confirming it is real. Researchers have been able to measure symptoms. They have determined that chemo brain is a very real side effect of chemotherapy.

Some reported symptoms include problems in the following areas:

- Memory. Weakened short-term memory. Memory lapses. People's names escape you. Cannot remember where you put your keys, remote, glasses, etc.

- Concentration. Can't focus. Easily distracted. Cannot read an entire book.

- Multi-tasking. Can only concentrate on one thing at a time. Cannot talk on the phone while cooking without losing track of one or the other.

- Word Recall. Cannot find the right words. Cannot finish sentences because the right words elude you.

- Slower Thinking. Cannot learn new things as fast as before. Takes you longer to finish things.

The symptoms listed here are the most commonly reported. Sufferers can experience a multitude of issues not present prior to chemo. Some symptoms can be mild and some fierce. The symptoms can last anywhere from a few weeks to ten years.

As you can imagine, this is very frustrating for the sufferer. Cognitive dysfunction can seriously affect a person's quality of life. I do not know how someone with this condition could not develop coping strategies. Some include:

- Using day planners can be a godsend. Keeping everything in one place makes tracking easier. You can enter ideas, promises made, appointments, phone numbers, etc.

- Routines can be helpful. Put your keys in the same place every time. Keeping a daily schedule can ensure that daily tasks are completed.

- Do not try to multi-task. Only try to focus on one thing at a time.

On average, symptoms disappear in about four years. However, some things you can do may help the process. Lifestyle factors may help overcome cognitive dysfunction. For example, exercising your body and eating vegetables.

Exercising your brain can also be very helpful. There are countless stories of people who have put their lives back on track after suffering mental dysfunction. This includes people with such issues as traumatic brain injury, strokes, and yes, chemo brain.

Challenging your mind is one of the best ways to overcome cognitive dysfunction of any type. There are brain-training games out there that have helped others with chemo brain. They are worth investigating to see if you too can defeat chemo brain.

2. Stop Eating ALL junk foods ~ today.

The goal here is to stop the inflammation of the intestines. That's only going to be done by putting the right stuff in and getting the junk out. Processed foods inflame and irritate the intestines. Sometimes people "think" they are eating well, but if you suffer with either of these two conditions, you need to avoid eating things like pizza, rice, cereal, pasta, white potatoes, bagels, cookies, crackers, (basically dead foods!) And forget pasta sauce! Too acidic ~ just for now until you heal.

3. Eat healthy foods that are non-irritating to the digestive tract.

A smoothie for breakfast would be ideal (containing strawberries and banana, or pineapple and banana, coconut milk, coconut water, and protein powder). This will be high in nutrition and non irritating to the digestive tract. The good fat from the coconut milk with help if you want to keep weight on.

Lunch can consist of canned or wild-caught fish such as salmon, or grilled chicken, over lightly cooked green veggies with olive oil such as spinach, kale, or any other green vegetable you enjoy. You can also do salads as well (with extra virgin olive oil and some lemon juice).

Supper can be similar to lunch. Remember, it's only limiting for a while until you are feeling better. It can take 6 months to a year to recover fully, but it's so worth it! You will adapt a healthy lifestyle to boot!

4. Stay Away from ALL Alcohol and Coffee Right Now.

This is critical, as both alcohol and coffee are super acidic to the body and contribute to inflammation. Enough said on that one!

5. Chew Your Food Very Well.

This is critical to your success because again, the inflammation must come down. You want to put as little strain on your liver as possible, and you want to put the best foods in your system that will nourish you, and not rob you of health. Chewing your food well will create enzymes in your saliva to help you break down your food so much easier. Then the liver won't have to work as hard to break the food down, and it will go into the intestines much easier.

Teeth whitening has become extremely popular over the last several years due to the availability of at home teeth whitening systems.
The first time I got my teeth whitened was at my dentists office. I had to sit there for about and hour as they installed the equipment in my mouth and applied the whitening agent. What Every Cancer Patient Must Know About Sunlight, Vitamin D and Cancer

Everyone who has been diagnosed with cancer must know this information. Please read the whole article because it may save your life.
Cancer cells are immature cells that divide uncontrollably. These cells can form tumors and break away from their tissue of origin causing the cancer to spread to distant sites (aka- metastasis) through the blood stream or lymphatic tissues. Where do these immature cells come from? One answer is quite simple. The lack of adequate sunlight.
The people of the world have been told that the sun causes skin cancer. This is true if you have been sun burned. Sunburns can cause skin changes that may lead to skin cancer. But did you know that the lack of sunlight can increase you chances of getting of colon cancer, breast cancer and prostate cancer. In fact the lack of proper sunlight has been linked to 17 types of cancer.
Why does sunlight help cancer? The answer is Vitamin D. Cholesterol in your blood is converted into Vitamin D when sunlight hits your skin. Vitamin D helps normal cells stick together (cell cohesion) through something called gap junctions. Gap junctions not only help your cells stick together but help your normal cells communicate with each other. Yes! Cells can talk to each other through special chemical messengers. Gap junctions are like tiny tubes that connect two cells together allowing for communication.
What do cells talk to about? Normal cells tell each other what they themselves are doing and what they expect their neighboring cells to do (like when a cell should divide, and when not to divide). Normal cell division takes place during growth periods (like when an embryo or child is growing) or to replace old or injured cells. You might say that when each of us was an embryo in our mother's womb, we were one single immature ("cancer like") cell that divided rapidly to become a living human baby in only nine months. These rapidly dividing embryonic cells did not become a cancerous tumor because the immature cells talked to each other through gap junctions. Through this chemical communication, some cells became specialized mature cells like skin cells, heart muscle cells, liver cells, bone cells etc. Cancer is when specialized mature cell revert back to their embryonic, immature state which leads to uncontrollable division and tumor formation. It is like when cells don't communicate, they get confused and go wild.
In a nutshell, Vitamin D deficiency (aka sunlight deficiency) leads to gap junction dysfunction, poor cell communication, poor cell cohesion (they don't stick together), mature cells reverting back to their immature embryonic "cancer like form", and tumor growth. Because of the poor cell cohesion, these cancer cells then break away and form new tumors in other parts of your body. This is why cancer can spread (metastasize) from one body organ to another.
Other Vitamin D facts:
1. You cannot eat enough vitamin D in food to help fight cancer. You would have to drink 50 glasses of milk a day to get enough vitamin D. Vitamin Supplements is a good way to increase your Vitamin D levels if getting proper sunlight is not possible. Vitamin D3 is the best supplement
2. Your body cannot make enough vitamin D from sunlight during the winter months if you live in northern states like oregon, washington, new york etc. Even people in warmer, sunny places can be Vitamin D deficient.
3. Ultraviolet - B rays (UV-B) from the sun are needed to make Vitamin D in your blood. Very few UV-B rays hit the earth in the winter months. UV-B rays do NOT penetrate through clouds, walls, clothes, and clear glass. Getting sun rays though clear glass will not help you make vitamin D.
4. Vitamin D is not toxic as once thought. Toxicity is difficult to achieve. (note: water is toxic if you drink too much)
5. Optimal levels of Vitamin D in your blood is 50-80 ng/ml (not 30 ng/ml as some blood labs suggest)
6. Taking 5,000 IU of Vitamin D3 per day is safe especially if you are deficient. Prescription-only Vitamin D is 50,000 IU's taken once per week. Get your blood tested for vitamin D every few month to determine if your blood levels have reached optimal levels (50-80 ng/ml)
7. Cancer patients need a lot of vitamin D to help in their fight against the disease. It can take months to build up your vitamin D levels in you blood to achieve optimum levels.
8. Tanning beds that create UV-B rays will help your body make Vitamin D. Short periods of tanning in a safe tanning bed is good for your health.

Most discussions regarding health insurance are based on issues like group versus individual plans and the type of insurance you buy whether it be HMO, PPO or what have you. Health savings accounts don't get much mention, but they really should. Why? They constitute a unique approach to health insurance, but one that works for many people.
Ask yourself a simple question. Why do you have health insurance? People tend to look at the question from two perspectives. The first group wants coverage for everything and is willing to pay for it. The second group just wants coverage in case something dramatic happens and huge medical bills are incurred. A health savings account is going to be of particular interest for this second group.
A health savings account often goes by the abbreviation "HSA", but is more often called a health IRA in the financial industry. This is because the program works much like an IRA. When you set up an account, you buy what is known as a high deductible health insurance policy. This deductible is often $1,000 and means that you must pay the firs $1,000 in medical bills incurred before the insurance kicks in. In exchange, the premiums charged for the insurance are much lower since the insurance company does not have to pay for the little charges most people incur.
Now things get really good. You are allowed to contribute a certain amount of money each year to the account to ostensibly cover the deductible amount and cost of insurance. In 2010, this amount is $3,050 for individuals and $6,150 for families. The contribution can either be made with pre-tax dollars through your employer or you can take a tax deduction if you do pay with post-tax dollars. The money is then used to pay the bills. Anything you have left over at the end of the year rolls forward to the next year. Slowly, but surely, the balance grows as you invest it over time. When you retire, the balance can be withdrawn just like an IRA. In short, you can pay for your health insurance, get a deduction and save money for retirement. If that doesn't sound good, nothing will.

To say health care and insurance has been in the news lately would be a minor understatement. The subject of reform has led to reactions ranging from its about time to its a socialist move. One aspect of the situation that is not really being discussed is an issue of federalism and it is important because it reflects a bigger trend.
What is federalism? It is simply a political structure in which there is a central political group and surrounding smaller groups. In the United States, this takes the form of the federal government and the individual states. The biggest issue in federalism is who has the power. In a country like Switzerland, the central federal government has very little power at all. In the United States, the opposite is the case but states are starting to contest this and the healthcare debate shows us a perfect example.
One of the aspects of the proposed healthcare reform effort is that all citizens will have to participate or be assessed penalties. These penalties would be collected by the IRS. The exact provisions are a bit murky, but the mandatory nature of the system has met with open hostility in a number of states. This has manifested in an interesting way at the state government level. No less than 35 states currently are considering legislation that will excuse any of their citizens from being forced to participate in such a national plan or pay penalties for not doing so. The vast majority, if not all, of these bills will pass.
So, what happens then? Well, we are going to be looking at a stand off between the federal government and states. Our current system calls for federal law to trump state law. Practically speaking, however, we see the federal government backing off in some areas such as medical marijuana prosecutions in states where laws have been passed allowing it. The question is whether the same will occur with a national healthcare plan? The answer is almost assuredly that it will not. You can't have part of the country forced to pay into the plan while some states do not.
All of this leads to the issue of where power should sit in our current system of federalism. With the federal government perceived to be out of control, you can be the issue will be moving to the front more and more as time passes.

Chemotherapy and Brain Damage. Is There a Link?

Many chemo patients report mental impairment. Some estimates say as many as 50%. They notice it during and after chemo. The impairment was not present before chemo. Is there a link between chemotherapy and brain damage?

Chemo uses chemicals to kill cancer cells. The body is pumped full of toxins. This would likely cause some cognitive deficits. Mental impairment occurs more often in those treated for cancer involving the reproductive system. Examples are breast and prostate cancers. In these cancers, the chemo levels are very high.

Some call the mental impairment a mental fog. Some call this condition chemofog. The term chemo brain came about in 2004. The term describes the mental fog patients feel during and after chemo treatments.

Researchers have known about chemo brain for over twenty years. Since the 1980's, there are a large number of breast cancer survivors. Many of them have reported symptoms. They started to suspect issues with chemotherapy and brain damage.

They were reporting problems they did not have prior to chemo. Problems with their memory. They were fumbling for words in the middle of sentences. They could not concentrate like before. They could not multi-task any longer. They were having a hard time learning new things. These are typical reports from a chemo brain sufferer. They were feeling half-crippled.

These symptoms lasted for a few weeks to years. Survivors reported different levels of each symptom.

Only recently have there been studies involving chemotherapy and brain damage. The medical profession did not understand the phenomenon. Doctors trivialized the condition for years.

The cause of chemo brain still is not clear. It is not clear how often it happens or what actually triggers it. Some theories are that stress, fatigue and depression causes the effects. Hormone changes, lack of estrogen.

When talking about chemotherapy and brain damage with patients, they have differing views. Some patients have extreme symptoms. Some notice no issues. They are not always sure if they have chemo brain or age-related issues. While other cannot notice a difference before chemo than after chemo.

Medical professionals also have conflicting views on chemotherapy and brain damage.

Some say that the link is real. They point to the large number of documented chemo brain cases. There are also a growing number of studies by researchers. These studies are showing that mental impairment after chemo is real. They are also showing that the impairment can last longer than two years.

In one study, researchers took brain scans of breast cancer survivors. They used chemo patients and patients who survived cancer through surgery. The study showed the brains of chemo patients had to work harder to perform the same tasks.

Others argue the stress of chemo could have an impact on mental function. They also cite reduced levels of estrogen, fatigue, and anxiety. These could all cause chemo brain symptoms. They also point to other medications. Chemo patients often take other dugs to treat chemo side effects.

Another study on chemotherapy and brain damage had a different conclusion. They discovered there might not be a link between chemotherapy and brain damage after all. This study evaluated the cognitive state of women before chemo. Thirty five percent of the women showed some level of mental impairment. Memory was impaired most often.

The conclusion was twofold. First, the cause of chemo brain might be due to factors not considered yet. The second is that any study linking chemotherapy and brain damage must include tests prior to chemo.

The debate about chemotherapy and brain damage will continue.

The bottom line is that the cause of the mental impairment does not matter. What matters is that people are suffering. Chemo brain, cognitive impairment, chemofog, what ever you call it. It has a direct impact on a person's quality of life.

I am sure you have heard of colon cancer, but do you know what it is and do you know what causes it? Colon cancer - also known as colorectal cancer - is the unrestrained expansion of cancer cells on the inner lining of the colon. Now if you forget what a colon is (which is normal, since we all aren't medical students), it is simply the part of the large intestine that extracts moisture from one's digested food before it is excreted. A common ailment in Western societies, this type of cancer has taken the lives of many, resulting in broken distraught families.
Now let's take a closer look at what causes this terrible disease. Notably, we owe this knowledge to the biochemist Dr. Warburg who discovered the cause and prevention of this type of cancer. He states that plainly, colon cancer is caused by the lack of oxygen reaching the colon cells of the body. Developing later in this process are adenomatous polyps which also turn cancerous and in effect worsen the patient's health. These polyps are seen on the inner walls of the colon.
In addition, there are ways in which the lack of a healthy lifestyle can also increase the likelihood of developing colon cancer, which usually affects both men and women before the age of 40 years. These include smoking, which is a serious risk factor as it not only affects the lungs of the body, but many other systems as well. Obesity is another risk factor due to the fact that having a low oxygen level is prevalent in persons who are overweight. It therefore stands to reason that if a person can maintain a balanced oxygen level suitable for a healthy body and mind, then they can also decrease their risk of having a cancerous colon. This should be supplemented with a diet high in fruits and vegetables.
There are also risk factors associated with genes. In essence, adenomatous polyposis syndromes often run in families, making it much easier for a descendent to develop a cancerous colon, especially in cases with a first-degree family member with this illness. These risk factors are propelled even further if more than one relative have had the battle with colon cancer, developing it at an early age. Once the presence of colon cancer has been diagnosed, there are important additional factors which may increase its impact on the human body. Colon cancer cells use sugar as a source of energy to survive within the body and if a lot of sugars are part of the diet, then it can only pose more health problems to the patient.
All in all, there is a bright side to this grim picture; colon cancer - even though it is the second leading cause of cancer deaths in the Western world - is treatable, and more importantly, preventable. Even for those individuals who are at risk because of family history, they also can take preventative measures to decrease its chances of occurring. The best thing you can do is to always know your status, live a healthy lifestyle, and stay on top of your healthcare!

Finally, as the colonoscopy has been proven to be the best tool employed for early detection of cancer of the colon, women should know all about such a procedure. A colonoscopy gives doctors a clear view of the lining of the colon to identify any signs of growths or enlargements resulting in this cancer. This 15-30 minutes examination involves the insertion of a slender, supple scope with a camera and an illuminator into the anus. With a running cost of $500 to $1000, the patient is made to take a number of pills or to drink a lot of a special fluids to release the bowels beforehand so that when the individual is sedated and examined, a clear depiction of what is going on inside the colon can be obtained.
There is really nothing to be scared about while going through this process. You will normally be sedated and made as comfortable as possible while you are examined. Besides, it is all for a good cause, which is maintaining your health. Because colon cancer is becoming so common in women, it is better to be in the know than to be clueless until it is too late. Be proactive and get yourself tested today!

Will Remission Last

For me, yesterday was a bad day. For the fourth time in 23 years, a doctor told me that my PSA was on the rise. Each time that happens, I get that feeling in my gut that the fight is about to start over because the remission has ended. When will it end? I start considering my options on what I need to do next. Will it continue to be a non-traditional method or will I accept chemotherapy. I know in my heart that chemotherapy is out because of the massive damage it does to one's immune system.
It wasn't but a few hours later that I get an email from a co-worker that his wife has been diagnosed with poorly differentiated metastases breast carcinoma. Not being a doctor or schooled in medicine, sometimes it goes over my head, which is okay. Approximately seventeen years ago, she had breast cancer and apparently one or more of the cancer cells were missed and they settled in her L1 vertebra. Thus her battle is about to start over again.
This is a daily play that is replayed over and over again all across America as well as the rest of the world. We look at the cancer patients and give them all the support we can possibly give them. However, cancer is something that not only affects the patient, but family and friends as well. Plans and schedules are put on hold and sometimes careers and lives are totally decimated.
Hearing that terrible sentence is a reason to plan a one person pity party. Believe me, I know. I have had these little one person get togethers. Each time, I realize that the cancer may have taken up residence in my body, but it also rips at the fabric of the family and the strength of friendships. At these times, I realize just how fortunate I am to have a very strong belief in our Heavenly Father. I could not imagine going through a fight with cancer without my Eternal Friend.
Please do not interpret this to say all cancer survivors will have a point where their remission will end and the fight will continue. That is not the point. The point is that regardless of how well we manage our lives, our diet, and our way of life, if you are a cancer survivor, the remission could end at any time.
We have another friend that has announced that she will not take any more chemotherapy or radiation because of the advanced stage of the cancer that has all but destroyed her body. One thing that you will note is that it is my opinion that the cancer is not mine or yours. Cancer has simply selected me as its host. Unless there is a real slip up, my phrase will always be that the cancer that resides in my body or terms such as that.
When will the remission end? Hopefully never. For those of us that are facing the end of a period of remission, we need to look at it as though the first quarter of this game has come to an end and we have a lot of time left in this game. Arm yourself with all the knowledge and resources at your disposal and have a short battle and a long remission. There are a lot of fish to catch. B and E, we wish you well in your battle.
